Information for Patients

Booking an Appointment
  • You may book online or call the office at 613-687-7641

  • When calling in, please give the receptionist your first and last name, date of birth and reason for the appointment

  • It is important for you to give the receptionist some information for the appointment so they can book the appropriate amount of time, failure to do so may result in needing a second appointment or rescheduling the original appointment

  • If you wish to have a video appointment but you don't have access to a device, please let the receptionist know at the time of booking. You may come into the clinic for the appointment and we will set you up in a designated room

Preparing for your In-Person Appointment
  • Ensure any forms or questionnaires that were emailed to you are complete before your appointment

  • Please arrive 10 minutes early for your appointment in case there are any forms you need to fill out that were not emailed to you prior to the appointment

  • Check in at reception upon arrival. The receptionists will ask for your health card and confirm your address, phone number, email address and pharmacy to make sure everything is up to date

Preparing for your Virtual Appointment
  • Please leave a window of time before and after your appointment time in case your doctor or nurse practitioner is running ahead or behind

  • Do not call the office right away if you haven't been contacted by your doctor or nurse practitioner as they may be trying to contact you and you may miss their call

Video appointments

  • A link will be emailed to you, make sure your audio and video are working, you do not have to download any programs or create an account for a video appointment

  • If you do not get a link or you have accidentally deleted the email, please call us and we can resend it to you

Phone appointments

  • Make sure your phone accepts unknown and private numbers

  • If you were instructed to submit pictures/forms, make sure you have sent them in before your appointment so they are in your chart

  • Ensure you are in a secure area with good cell phone service 

Cancelling your Appointment/ Cancellation List
  • Please give us 24 hours notice if you cannot make it for your appointment or a No Show/Missed Appointment Fee may apply

  • We do not keep a cancellation list. For the most up-to-date appointments you may check our online booking or call the office for any cancellation inquiries

Missing an Appointment
  • If you miss your appointment, No Show/Missed Appointment fees may apply

  • Please be aware that if you arrive late, it will be up to the provider’s discretion to decide if they can accommodate you based on their schedule

Immunizations
  • To obtain a copy of your or your child's immunizations, you can do so here

  • Follow this link to view the list of Ontario's routine immunization schedule or this link to view the list in a table format

E-Mail Communication
  • We can communicate with you through email, please provide your email address and the first thing we will send is a consent to communicate electronically form

  • Any emails from the PCFHC come from "Petawawa Centennial Family Health Centre" with the email address "no-reply@cognisantmd.com". If you don't see the email in your inbox, please check your junk mail and remember to mark it safe so it goes into your inbox in the future

  • We are able to send you any questionnaires or forms required for your upcoming appointment, please have them completed prior to your appointment

  • If multiple patients use the same email address please enter the date of birth of who the email was addressed to 

  • This is a one way email communication system where the PCFHC can email you. Patients cannot initialize an email to the clinic through this way, although, you may reply if the provider/admin selects that option for you to reply

Chart Transfers
  • With a signed release of information, we can securely transfer your medical records to your new provider. Your new provider may not want all of your previous records right away, so we'll work with them to ensure a smooth transition

  • There is a fee for this service, please call our office to inquire

Picking Up Documents
  • If you had forms completed, please wait for admin to call you to notify you that they are ready for pick up. You may have had your appointment with your provider, but there are a few steps afterwards that need to be taken place

  • Make sure you bring a valid piece of photo ID with you when you come to pick up any documentation

Emergency Situations
  • If you have a medical emergency, please call 911 or visit your nearest emergency room immediately. Our office is not equipped to handle major health emergencies

Artificial Intelligence (AI)
  • With your consent, your health care provider may use Artificial Intelligence (AI) to record your conversation and create a detailed note of the clinical encounter. The doctor or allied health professional then reviews this note to confirm its accuracy and adds it to your medical chart. Once this is complete, the recording is deleted. All of your information is kept confidential and secure, just like the information in the rest of your chart. The computer program, AutoScribe, allows your healthcare provider to focus on you and not on the computer.

Misc
  • Please note that our receptionists do not have medical training and are not qualified to answer your health questions, provide medical advice or go over test results. They may advise you to book with your provider or speak to our nurse

  • We are currently not taking any new patients. We do not keep our own wait list, we take our patients from Health Care Connect (HCC). Please contact HCC to go on the provincial list
