Arrive on time for your appointment
Update your health card before it expires as there may be a fee if you have an invalid health card for your appointment
Inform us of any changes to your address, phone number or email address so we can still contact you.
Inform us of any changes to your selected pharmacy
Ensure you leave yourself enough time for your medications to be renewed before you run out
Prescription Refills
•Please contact your pharmacy to have them fax us a refill request•
•We require 7 business days for a prescription to be refilled•
Narcotics
•In order to refill this type of medication, your provider will want to speak to you first, please book an appointment before you run out, to discuss. When booking, please let the receptionist know what type of medication it is•

Patient Responsibilities

Arrive on time for your appointment. We recognize that circumstances may lead you to be late for your appointment. If that happens, it will be up to your provider's discretion to determine your ability to be seen. Our reception team is here to help make your experience as pleasant as possible. We kindly ask that you remain patient and respectful towards them; they are invaluable in facilitating your care

Prescription Refills

You may need an appointment if you are refilling a medication for the first time that your current provider didn't prescribe
Controlled Substances/Narcotics
In order to refill this type of medication, your provider will want to speak to you first. Please book an appointment prior to running out of medication, to discuss. When booking, please let the receptionist know what type of medication it is
Forms

Be aware there will be a fee for any forms to be completed by your provider as this is not a service covered by OHIP

Please sign your form. This indicates consent for your provider to fill it out

Drop your form off to reception. An admin in our Forms Department will look over the form and will contact you if an appointment is needed

You will receive a call from admin when your forms are ready to be picked up

Please note that rates may change depending on the services provided. We encourage you to discuss any concerns regarding fees with your health care provider
Miscellaneous

You must be located in Ontario for your virtual appointment. Our clinicians are not licensed to practice outside of Ontario
