Collaborative Care Midwifery Clinic (CCMC)
The Collaborative Care Midwifery Clinic (CCMC) will provide:
-
Prenatal care from intake to 28 weeks. The Registered Midwife will be able to offer full prenatal care for the first and second trimester. At 28 weeks they will be referred to obstetrical care at Pembroke Regional Hospital. Any patient who is determined to be high risk will be referred to obstetrical care earlier.
-
Postpartum care: for mom and baby for the first 2 months. Postpartum midwifery care includes home visits, newborn care, breastfeeding support, referrals for newborn concerns, monitoring well-being of mother, and Paps. During this time effort will be made to secure a permanent care provider for the newborn when discharged from CCMC.
This program is funded by the Ministry of Health through the Expanded Midwifery Care Model program. The goal of the CCMC is to relieve the current strain on health care providers in Renfrew County while offering excellent midwifery care to a larger number of women and their families.
